We know you’ve worked hard on the SMART Goals you created last year, and we want you to continue! Elevate Program Plans must be updated and approved annually to maintain the Member+ Level of Engagement.
Now until November 20, 2024, the first 100 programs with approved Elevate Renewal Plans will receive a one time $1,480 bonus.
The Elevate Renewal Plan is similar to your original Elevate Program Plan. It’s just an update! Your task is to reflect on last year’s goals and create updated goals for the year ahead.
1. Reflect on last year’s SMART Goals
Think about how your goals changed over the year. You can view your current Elevate Program Plan and SMART goals in the OEC 360 Provider Portal by logging in and going to your Elevate tile. Log into the OEC 360 Provider Portal now.
- Were there challenges you did not expect?
- What will you do differently next time?
- Which OEC resources did you find useful?
- If you achieved your goals, what got you there?
2. Create new SMART Goals for growth
You may develop new goals for your program on your Elevate Renewal Plan, or you may still need to continue your work on last year’s goals. Both paths are ok! The ultimate goal is to keep making progress toward quality in your program.
