Family Connects is an evidence-based universal approach to reach all families of newborns, including foster, adoptive, and bereaved parents, to offer a free, nurse home-visit for a maternal and child health screen and connection to local services to meet their unique needs.
Family Connects nurses are trained to carefully assess newborns and mothers and discuss concrete next steps to address opportunities and concerns, including seeking immediate medical care when necessary. FCI nurses also keep the whole family in mind, recommending appropriate community-based resources for other family members as needed — and they follow up to make sure families’ needs are met.
Family Connects Bridgeport is currently available to all families of newborns delivered at either Bridgeport Hospital or St. Vincent’s Medical Center, and living in Bridgeport, Easton, Fairfield, Milford, Monroe, Shelton, Stratford, and Trumbull.
