Annual Immunization Survey

Child care providers continue to play an instrumental role in assuring that Connecticut children are age-appropriately immunized. All licensed child care centers and group child care homes are, once again, asked to complete this survey. The annual immunization survey is administered by the Immunization Program of the Department of Public Health in collaboration with the Child Care Licensing Program of the Connecticut Office of Early Childhood.
